Life Support Assistance Program
Azusa Light & Water’s Life Support Assistance Program offers electric customers that require the use of Life Support Equipment in their home a reduced rate for the monthly electric service charges.
Life Support Assistance Program Eligibility:
To apply for the Life Support Assistance Program applicants must meet the following conditions in order to qualify for the assistance program.
1) The Azusa Light & Water account holder must dwell and claim the residential premise as a primary residence and complete the application.
2) The account holder or household member requires the use of one or more of the qualifying life support devices or medical conditions*.
3) Provide a Physician Statement of Certification completed by the patient’s primary physician.

How to apply for the Life Support Program Application

Once the application is submitted please allow 4 to 6 weeks to process the program application. If the application is approved please allow an additional one to two billing statements for the exemption to reflect on the ALW billing statement.
